1. Trimethoprim sulfamethoxazole 15-20 mg/kg/day divided 3-4 times per day for 21 days (IV for severe disease - increased work of breathing or A-a gradient >45 or PaO2 < 60)

If sulfa allergic then trimethoprim-dapsone, clindamycin-primaquine, atovaquone, or pentamidine

Add steroids if severe with PaO2 <70 or A-a >35. 40 mg prednisone BID taper over 21 days
