Side effects of 1st Gen (typical)
EPS (Extrapyramidal symptoms which is tardive dyskinesia which is uncontrollable facial movements, Dystonia, Akathisia, etc.), sexual dysfunction, robotic movements, hypotension, photosensitivity.
Side effects of 2nd Gen (atypical)
-Weight gain (especially Olanzapine, which is used for anorexia)
-Metabolic syndrome (high BP, BG, increase in body fat, etc.).
-Sexual dysfunction, hypotension, and photosensitivity.
-Clozapine has a side effect of agranulocytosis (Low WBC).
-Typically treats negative symptoms.
Treatment for EPS
-Anticholinergic/ Antihistamine
-Benztropine (Cogentin)
-Diphenhydramine (Benadryl)
Key information: Used to treat EPS side effects and triggers sympathetic nervous system (dry mouth, tachycardia, blurred vision, less urine production, etc.).
SSRI’s (Antidepressants)
-Fluoxetine (Prozac)
-Sertraline (Zoloft)
-Paroxetine (Paxil)
Key information: SE are sexual dysfunction, insomnia, nervousness, serotonin syndrome (Flu-like symptoms), and suicidal ideation. Also used to treat anxiety disorders and bulimia nervosa.
SNRIs (antidepressant)
-Venlafaxine (Effexor)
-Duloxetine (Cymbalta)
Key information: Also used to treat anxiety. Cymbalta is also used for neuropathic pain.
NDRI (antidepressant)
Bupropion (Wellbutrin/Zyban)
Key information: Also used for smoking cessation.
SARI (antidepressant)
Trazadone (Desyrel)
Key information: Also used for insomnia (depending on dosage for the pt). SE: Priapism
Tricyclic (TCA) (antidepressant)
Amitriptyline (Elavil)
Key information: SE are sedation and Orthostatic hypotension. These patients will also need occasional EKGs. Can be used to treat anxiety.
MAOIS (antidepressants)
-Tranylcypromine (Parnate)
-Phenelzine (Nardil)
Key information: Prescribed sparingly due to interactions with Tyramine in food (such as cheeses, cured meats, etc.)and other drug interactions. At risk for hypertensive crisis.
Herb (antidepressant)
**St. John’s Wort
Key information: Avoid the use of other antidepressants, alcohol, pregnancy, or sunlight. Discontinue 1-2 weeks before surgery.
Mood stabilizers (Lithium)
-Trade name: Lithobid/Eskalith
-Therapeutic range: 0.5-1.5
-SE: polyuria, hypothyroidism, and leukocytosis.
-Avoid: diuretics, dehydration, extra salts/fluids
Anticonvulsants
-Valproic acid (depakote/depakene)
-Lamotrigine (lamictal)
-Carbamazepine (Tegretol)
-SE: drowsiness (will subside in 3-4 days)
-Assess: valproic acid levels and liver function
Benzodiazepines
-Lorazepam (Ativan)
-Chlordiazepoxide (Librium)
-SE: dependence, RR depressions, sedation
-Treats anxiety, seizures, alcohol withdrawal
-REVERSAL AGENT: Flumazenil (Romazicon)
Antianxiety
-Buspirone (buspar)
-Hydroxyzine (Atarax/ Vistaril)
-Takes 2-3 weeks to be effective
-nonaddictive
-avoid alcohol and grapefruit juice
Disulfiram
Antabuse
-for alcohol abuse
-alcohol needs to be avoided 12hr prior to starting the medication; and wait 2 weeks after discontinuing Antabuse before using alcohol
