  3. identification and species of animal
  4. date. normal Rx valid for 6 mths. CD RX VALID FOR 28 DAYS.
  5. name, quantity, dose and admin instructions - AS DIRECTED NOT OKAY
  6. warnings and withdrawal period if necessary
  7. If human med is prescribed and not licensed for use in animals: “PRESCRIBED UNDER THE VETERINARY CASCADE”
  8. FOR CD2,3: THE ITEM HAS BEEN PRESCRIBED FOR AN ANIMAL OR HERD UNDER THE CARE OF THE VETERINARIAN
  9. if Rx is repeatable (not a CD) the number of repeats required.
    VETERINARY CD RX NEED TO BE RETAINED FOR 5 YEARS AND NOT SENT TO NHS AGENCY.
