How many years should veterinary records and responsible pharmacist records be kept for?
For 5 years

Who must the peer discussion for the revalidation records be with?
Anyone who can provide constructive feedback on your practice, such as a colleague, manager or patient.
Can be a non-healthcare colleague.
Who cannot be a peer for the peer discussion of the revalidation?
- People you have a close personal relationship and either of you feels they cannot be objective
- People you have a conflict of interest with, or a perceived conflict of interest, like a commercial relationship
- They are a health professional, currently under sanction from a regulator
What needs to be reported to the MHRA / yellow card scheme?
- Black triangle medicines: Report all ADR even those that are not considered serious
- ADR to vaccines
- Established medicines that has caused harm, or is medically significant like life-threatening or causes hospitalisation.
- ADR due to misuse of medicines
- Falsified medicines
-Delayed ADR - e.g. cancer - Defective medicine
- ADR to medical devices like IUD
- Safety concerns for e-cigarettes and e-liquids
What are black triangle medicines?
Medicine that has been newly released; and is under pharmacology vigilance
