OTC for Ulipristal?
- Ulipristal is licensed for any woman of child bearing age, including adolescents.
- It is only licensed for use within 5 days (120 hours).
- If the patient is taking any CYP450 enzyme inducers such as carbamazepine, it should be prescribed by a doctor, and not given over the counter.
- It should not be used in pregnancy, and if the mother is breastfeeding, a one-week break is recommended.
What defines scabies?
Scabies is defined by an itchy rash caused by mites.
Symptoms include a raised rash or spots which spreads across the whole body, apart from the head and neck.
It often affects skin between the fingers, around the wrists, under the arms, and around the waist, groin and bottom.
Tiny mites lay eggs in the skin, leaving lines with a dot at one end.
If a patient potentially has Cystitis and also has hypertension. what can be given to treat their Cystitis?
What must be avoided?
Sodium citrate should be avoided in hypertension, BUT potassium citrate can given.
As well as paracetamol, plenty of fluids and a hot water bottle over the lower abdomen.
What counselling points can be given if a person is suspected of having Cystitis?
- Avoid having sex
- Avoid drinks which may irritate your bladder such as fruit juices, coffee and alcohol
- Pee frequently.
- There is no evidence that cranberry can help once infection has already started.
What are the dos and don’ts on scabies?
- Do wash all bedding and clothing in the house at 60C or higher, and if possible in a hot dryer, on the first day of treatment
- Do put clothing that cannot be washed in a sealed bag for 3 days until the mites die
- Don’t have sex or close physical contact until you have completed the full course of treatment
- Don’t share bedding, clothing or towels with someone with scabies
How long must you wait to go back to work or school if you have scabies?
People can go back to work or school 24 hours after the first treatment.
Someone is set to go to a hot country. He wanted to purchase some insect repellent which contains DEET and then a high SPF sunscreen. How do you apply the two?
Apply insect repellent about 20 minutes after applying sunscreen to give sunscreen time to soak into your skin.
Avoid products that combine sunscreen and insect repellent into a single package as sunscreen needs to be applied more frequently than DEET.
What is rhinitis medicamentosa?
A type of nasal congestion that occurs when you overuse nasal sprays.
Caused by prolonged use (>7 days) of topical decongestants)
How to treat a stye?
Use a warm compress for 5-10 mins four times a day.
Applying a warm compress allows the follicle to open and the pus to drain.
NICE guidelines do NOT recommend topical antibiotics such as chloramphenicol unless the pus is drained out.
What cannot be given if the patient has an ear wax build up but is allergic to peanuts?
Arachis oil CANNOT be used.
