What are normal sterile body sites?
Blood, cerebrospinal fluid, vitreous fluid, joint (fluid/tissue)
Which body sites have bacterial flora?
Skin, secretions from the body, drains
When should swabs be taken?
When there are clinical signs of local infection (pain, pus, abscess) or symptoms/signs of systemic infection (viral respiratory)
What are common screening samples for MRSA?
Nose, axilla, groin
What tests are required for screening?
Screening - MRSA, aerobic culture, anaerobic culture, PCR - DNA/RNA of organisms
What swab should be used for viral culture?
Viral transport media (VTM)
What swab should be used for anaerobic culture?
Gel-swab
What swab should be used for PCR?
Dry swab/Universal Transport Media/VTM
What swab should be used for aerobic culture?
Dry swab/gel-swab
How to sample an exposed wound with signs of infection?
Clean away debris, deep swab is better, swab with firm pressure ~1 cm², rotate the swab, press into wound to obtain fluid
What is the common swab type for bacterial culture?
Rayon bud with plastic shaft and Amies transport medium
Suitable for aerobic culture, anaerobic culture, screening (e.g., MRSA). NOT suitable for bacterial PCR, viral PCR, viral culture.
What is the common swab type for viral culture and viral PCR?
Flocked swab with reddish UTM/VTM (Red cap)
Provides supportive environment for viruses, chlamydia, mycoplasma. Antibiotics to prevent bacterial overgrowth. Suitable for viral culture, viral antigen, viral PCR, chlamydial PCR. NOT suitable for bacterial PCR, aerobic culture, anaerobic culture.
What is the common swab type for bacterial culture and bacterial PCR?
Flocked swab with liquid Amies media
Suitable for aerobic culture, anaerobic culture, bacterial PCR, screening (e.g., MRSA). NOT suitable for viral PCR, viral culture.
What is the procedure for urine collection using a catheter specimen?
Clean sampling port, sample from the sampling port, aspirate the required amount of urine into the syringe, inject urine into specimen pot
How to collect sputum?
Ask patient to hold breath a few seconds, then cough directly into the specimen container
What is the turnaround time for non-culture tests?
Blood tests: 1-3 days, microscopy: 1-2 days, PCR: 1-3 days
What is the turnaround time for culture tests?
Aerobic culture: 2-5 days, viral culture: 7-21 days, TB culture: 10-42 days
What does a sterile site indicate?
Infection
Contamination - grow organisms that are not intended site culture
What does a non-sterile site indicate?
Colonization - growing organisms that are unlikely to cause infection or growing organisms without signs of infection, contamination, infection
