Outline the treatment of acute migraine. (DU-22M)
Mention the drugs used in acute attack and prophylaxis of migraine headache. (DU-14Ju)
A) Drugs for acute migraine headache:
Migraine specific:
Triptans (5-HT1 agonists): Sumatriptan, Rizatriptan, Zolmitriptan, Almotriptan
Ergot alkaloids: Ergotamine and ergonovine.
Non-specific (symptomatic):
NSAIDs: Paracetamol, Aspirin, Diclofenac, Ibuprofen, Naproxen.
Anti-emetic: Prochlorperazine, Domperidone, Metoclopramide.
Corticosteroids.
Opioid: When other treatment are not successful.
B) Drugs for prophylaxis of migraine:
1.β-blocker : Propranolol, nadolol.
2.Tricyclic anti-depressant : Amitriptyline.
3.Anticonvulsants :Na-valproate
4.Calcium channel blocker : Verapamil.
5.Anti-histamine & 5-HT antagonist : Pizotifen.
Q. Mention mechanism of triptans in reducing acute attack in migraine. [DU-23M]
Triptans( rizatriptan, zolmitriptan, sumatriptan)
They selectively activate 5-HT receptor which constricts the dilated cranial extra cerebral blood vessels
