Purposes of medication
- Prevent or treat disease or illness
- Treat symptoms
- Aid in diagnoses
- Restore or maintain normal functions
- Reach desired or therapeutic effects
Monitoring
- Delivering medication container to client
- Observing client to make sure medication were taken
- Reminding a specific client to take medications
- make a written recorded of each medication with note “monitored
self administration
- keeps medication in room
- no documentation need by client or Qmap
- care plan includes self-administration
- encourage client to check the expiration date
- best practices have a list of medications client is self administering
- obtain an order for each medication client is self -administering ( only is the facility administers some medications and client would like to self-administer other medications)
Administration
- Hand client staff prepared medications
- assisting client with medication

Qmaps can not administer
- Through gastrostomy tubs
- Through a nasogastric tube
- by injection ( epic pen/ insulin syringe)
Qmaps are not authorized
Perform assessment or make judgments
Qmaps cannot administers PRN that
Can’t give meds that change behavior ( I.e. anti- anxiety meds)
Unless clients capable of understanding and requests medication
All medications including over the counter medication
- much have writes orders
- be documented when given on the MAR at time of administration
Right medication
- only use trade or generic medication
- Have a drug reference guide or internet access to a drug reference site
- not administering a medication is a medical error
Trade name medication
The manufacturer or marketed name of a mediation
Do the three checks
- when pulling the medication
- when pouring the medication
- when putting the medication away?
Right time
-Make sure mads are giving at right time
—> order for a specific time
—> general time AM or PM
- meds might be given before or after meals
- some med certain time and way for best effectiveness
Synthroid/ levothyroxine
Best taken on an empty stomach without food or other medications
Potassium
Is best taken after a meal
Lasix
Is best give early during wake up hours
Right dose
-ensure the delivery of the right dose of medication ordered
Strengthen
May require partial tablets or multiple tablets to fill the order dose
Right person
-Client look similar with Photo ID on MAR
- Hearing impaired or confused could lead to error
- alway ask client to state names
Right route
- Oral and sublingual
- In eye, ear, nose
- Inhaled in mouth
- Vaginal or rectal
- Topical and transdermal
Right documentation
- MAR is a legal document
- only use black ink
- don’t use white out/ draw a line through error
- Mark Medication administered
- Medication not administered —> write reason on the back of MAR include ( not available, client out of facility, client refused)
Medication not administered
Write on the back of MAR reason not administered e.g. not available, client out of facilities, or client refused
Alway fallow facility policy and notify supervisor
Medication error
- Not reported medication was administered
- omitting a medication
- misinterpretation of the medication ordered or the directions
- transcribing order incorrectly on the MAR
- Documenting a medication was administered when it was not
Qmap moral obligation
Qmaps have a moral obligation to report medical errors
Clients may only self administer medication upon a write order
False
