what does serum light chain levels detect?
excess of light chains
what does immunofixation do after electrophoresis?
determines subtype of immunoglobulin
what are immunoglobulins produced by?
B cells (mostly plasma cells)
describe the structure of immunoglobulins?
Y shaped

the Fc portion is determined by what chain?
the heavy chain
what is the most common immunoglobulin ?
IgG
- 75% of total
what immunoglobulin is first to be produced and has the highest molecular weight?
IgM - pentamer

what are the two types of light chains?
kappa and lambda
what paraproteins are present in lymphoma?
IgM paraproteins
what paraproteins are present in myeloma?
IgG and IgA
does lymphoma or myeloma occur in the early stages of production?
lymphoma since IgM is made first before IgG and IgA
myeloma is disease of what part of the body?
the bone marrow
what age is most affected by myeloma?
70yrs
what is the definition of myeloma?
disorder of plasma cells that result in excess production of a single type of immunoglobulin
state some features of myeloma?
bone disease - lytic bone lesions, cord compression and hypercalcaemia
bone marrow failure (esp anaemia)
infections
state some systemic effects of myeloma?
renal failure - due to immunoglobulin deposition
hyperviscosity - causes bleeding, cardiac failure, pulmonary congestion and renal failure
hypogammaglobulinaemia
amyloidosis
what is amyloidosis ?
deposition of fibrillar protein
when amyloidosis is caused by paraprotein or light chains what is it called
AL amyloid
state some features of AL amyloid?
nephrotic syndrome cardiac failure carpal tunnel syndrome autonomic neuropathy cutaneous infiltration
