What is myQA iON and how is it used by clinicians?

A

myQA iON provides clinicians with a secondary dose calculation using an independent Monte Carlo dose algorithm and provides a 3D gamma comparison prior to the patient treatment.
Throughout a patient’s treatment, myQA iON also has the ability utilize the delivery logfiles that are generated by the treatment machine and perform a Monte Carlo calculation based on the delivered plan - pretreatment and per fraction. Version 2.1 will include the ability to launch and review the results from the Scandidos Delta4 within the iON platform.

What can treatments/modalities can iON be used for with the secondary dose calculation? For log file analysis?

A

a. Varian: photon, electron, SRS cones for TrueBeam (Millenium, Millenium HD120), TrueBeam STX (Millenium HD120), Halycon - (secondary dose calculation and log file analysis)
b. Elekta: photon, electron, SRS cones for MLCi2 (80 leaf) and Agility (160 leaf) MLC on Infinity, Synergy, VersaHD - (secondary dose calculation and log file analysis)
c. Accuray: Photon for Tomotherapy and Radixact with Precision - (secondary dose calculation only)
d. IBA: Proton for Proteous Plus and Proteous One - (secondary dose and log file analysis)

Does myQA iON have the ability to utilize golden beam model?

A

Yes, gold beam data is available for both Varian Gold Beam Data and for the Elekta AGL (Accelerated Go Live).

Any energy not listed in the specific table will need to be customized model. We also provide the option for customized models for energies that we do not have GBD for or for those who are not matched to the golden beam data and/or would like a specific fit to their machines.

Tomotherapy and Radixact we have a golden beam model, but for fine tuned models you will need to perform refinement validation - Dose rate, field edge correction, and time latency (optional parameter). We also provide the option for customized models.

What is needed for the modelling process?

A

For modelling, a site will be asked to fill out the beam data modelling sheet. This sheet requests:
* Treatment device data,
* absolute dose calibration data,
* measurements of:
- depth dose curves with detector information,
- output factors with detector information,
- crossline and inline profiles (including the maximum field) at various depths,
- and diagonals for maximum fields, and
* MLC calibration information

If for specialty models:
* SRS Cone Measurements
* Electron Applicator data
* Tomotherapy/Radixact
- Dose rate calibration database
- Latency correction database
- Dose rate calibration

If a customer is utilizing gold beam models (Varian or Elekta) they will only need to provide:
* Treatment device data,
* Absolute dose calibration data,
* MLC calibration information

Who performs the modelling?

A

Modelling is performed by IBA Service Support.
At the end of modelling, validation reports are provided to the customer to display the fit of the created model to the measurements that were used. The customer will also be asked to provide a clinical treatment plan (CT, SS, Plan, and Dose) so a calculation with their new iON model is then compared for each energy provided. This will help to establish what the customer can expect. If there are any concerns about the models or the customer feels they need to be corrected, the team will do so happily based on the customers input.

How long does a calculation take?

A

A Scimoca calculation can take anywhere from 1.5-5 minutes depending on the calculation settings. The user has the ability to manipulate the calculation grid size and the uncertainty.

How is the DLG/leaf modelling implemented in the SciMoCa models?

A

The MLCs can be modelled to fit the customers measurements. The DLG is applied to the leaves individually and uniformly. There is only one value that can be provided to the model per energy.
