typhoid fever presenting as fever (peaks and troughs), malaise, diffuse abdominal pain and constipation. Rose spots which are salmon colored, blanching trunchal maculopapules (1-4 cm wide < 5 in number) resolve within 2 -5 days. If left untreated the fever can progress to delirium, obtundation, intestinal hemorrhage, bowel perforation and death within 1 month of onset. Survivors may have long-term permanent neuropsychiatic complications. Name the causative agent.
Salmonella typhi
Severe, watery, non-bloody diarrhea and low grade fever. Occurs after eating contaminated meat or eggs. Incubation period 6-48 hours.
Salmonella
Associated with undercooked ground beef and unpasturized juice and milk. Severe abdominal cramping and bloody diarrhea. Fever is less common symptom.
E. Coli
Not a food borne related infection. Risk factors: antibiotic usage, advanced age, recent hospitalization, residing in a nursing home.
Clostridium difficile
From waterborne shellfish. Profuse watery diarrhea- “rice water stools” Fever is rare. Incubation period is 12-72 hours
