28
Q

What is the “CORE” of the NAVOSH Program?

A

Training, identification, control and elimination of safety and health hazards.

29
Q

What are safety inspections?

A

-Ensure the workplace’s are inspected ANUALLY
- Safety Officers retain inspection results for at least 2 YEARS

30
Q

What are Industrial Hygiene Surveys?

A

Each ship needs a basline survey
Baseline → Yard completion periods → New Construction → Periodic (every 3 years)

31
Q

What are some examples of IH surveys?

A

Executive Summary of findings
List of eye hazardous processes
Air sampling results
Hazard Evaluation
Medical Surveillance Requirements
etc.

32
Q

What are some examples of Renewing the Baseline inspection?

A
  1. New or modified equipment
  2. Introduction of a new toxic chemical
  3. Deterioration of existing controls
33
Q

What are Job-site Observations?

A

Detect and correct hazards resulting from worker non-compliance; if frequent can warrant a safety stand down :(

34
Q

Who can do a random walk through on a Job-Site Observation, and when do the typically conduct them?

A

CO
XO
Department Heads
Division Officers
Work Center Supervisors
( performed during the work day or during evolutions)

35
Q

What are Afloat Operational Safety Assessments (AOSA)?

A

Conducted by Commander Naval Safety Center
1-2 Days in Duration
Includes training and a survey of a sample of entire ship
Its purpose…
-Identify safety Hazards
-Trains safety personnel
-Provides CO with an evaluation of the ship
-Promotes hazard awareness

36
Q

When are AOSA’s typically done?

A

Can be conducted at command request
Every 6 years for Surface Ship and Submarines

37
Q

What is a Board of Inspection and Survey (INSURV) and Command Occupational Safety and Health Management Evaluations?

A

-Evaluates the Ships compliance with NAVOSH
-SOH inspections conducted during final contract trials
-Copies of all SOH related discrepencies are routed to the Safety Officer and are all tracked until verified as corrected.

38
Q

What are Civilian Officials?

A

-OSHA may conduct inspections on ships, but must meet ship requirements
-State officials MAY NOT conduct safety inspections on ships, they get executed on sight.

39
Q

What is the purpose of the Medical Surveillance Program?

A

-To monitor the health of individuals exposed to hazards in the fleet (by job certificiation and re-certification exams)
-Monitor health of employees previously exposed to hazards.
-Prevent actual disease, injury, illness.

40
Q

What are the requirements of a Safety Hazard Report?

A

-Hazards are tracked until verified as corrected or eliminated
-Some can be corrected by ships force, other during availabilities or maintenance periods
- Tracked and forwarded to Safety Officer

41
Q

Reguarding Safety Hazard Reports, if a supervisor fails to take appropriate action what can the crew member do?

A

Complete an OPNAV 3120/5 (Safety Hazard Report)
-Hand written or typed
-Forwarded to Safety Officer

42
Q

What is the Hazard Abatement Program?

A

Process that identifies hazards that are not able to be immediately corrected and are recorded and tracked to completion.
-Hazards are tracked until verified as corrected or eliminated.

43
Q

What is a Risk Assessment Code (RAC)?

A

Each identified hazards are assigned a RAC by the Safety Officer.
-Provides a priority for the corrections and deficientcies

44
Q

What is Hazard Severity?

A

Assessment of the worst reasonably expected consequence defined by degree of injury, illness or damage to occur.
-Catagorized by Roman Numerals I-IV

45
Q

What are the Hazard Severity Catagories

A

I: Catastrophic: Hazard may cause death or GRAVE damage to national interest
II: Critical : Hazard may cause severe injury or DAMAGE to national interest
III: Marginal: Hazard may cause minor injury
IV: Negligible: Hazard presents a minimal threat.

46
Q

What is Mishap probability?

A

The likelyhood that a hazard will result in a mishap
-Catagorized by Letters

47
Q

What are the catagories of the Mishap Probability?

A

A. Likely to occur immediately or the very near future
B. Probably will occur in time
