What does co-careldopa do

A

Levodopa and carbidopa
Carbidopa breaks down peripheral DOPA decarboxylase so levodopa can get into BBB
Otherwise DOPA decarboxylase will cause premature conversion to dopamine -> N&V s/e

WHat are the s/e of co-careldopa

A

General information
* Deteriorating function = worsening of symptoms over time
* Loss of drug effect = loss of drug efficacy over time
* Motor fluctuations = end-of-dose fading or ‘on-off’ phenomenon
* Dyskinesia = chorea or dystonia
* Freezing gait = inability to start/continue moving

Safety information
* Neuroleptic malignant syndrome = can occur if anti-Parkinson’s meds stopped abruptly (fever,
confusion, autonomic dysfunction and rigidity)
* Impulse control disorders = gambling, binge eating, shopping +/-risk to self/others
* Overuse of anti-Parkinsonian medication
* Delusions or hallucinations = belief that is not in line with cultural/religious norms OR sensory
perception without an actual stimulus

What is the management of myasthenia gravis

A

Pyridostigmine temporarily improves symptoms of MG (initial sx relief)

Pyridostigmine is a long-acting acetylcholinesterase inhibitor that reduces the breakdown of acetylcholine in the neuromuscular junction, temporarily improving symptoms of myasthenia gravis

How do you manage myasthenia crisis

A

intravenous immunoglobulin, plasmapheresis (remove ai antibodies) IV immunoglobulin (more likely to form clots if given so not given to pt who are at risk of clots)
