Nomograms Flashcards
(33 cards)
Nomogram definition
figure or equation consisting of three or more straight or curved lines, each graduated for a different variable - specific mathematical relationship
BSA DuBois
S = W^0.425 X H^0.725 X 71.84
w- kg
h- cm
s- Cm^ 2
cm to m
100 cm in one m
Kest
aminoglycoside dosing, emperic dosing,
0.0024xCrCl + 0.01 hr-1
Vest
0.25 L/kg X IBW kg (use ABW if ABW < IBW)
average aminoglycoside t 1/2
2
how does the drug damaging the kidney effect
decreases clearance and causes toxicity
when to use ABW female
ABW if ABW < IBW
when to use ABW
use ABW if ABW <25
why we use nomograms
maximize efficacy and minimize toxicity
MIC
minimum conc in body to kill bacteria but no resistance
dose of aminoglycoside
7 mg/L
target peak aminoglycoside
20-30 mg/L
trough aminoglycoside
trough undetectable
peak vs MIC
peak is 10X in MIC
why trough negligible
toxicity comes from the trough, accumulation, ODO and renal toxicity
drug free interval adjusted
about 6
when to take a random
after start of conc, tells what interval pt will be on moving forward
adjust the interval when and based on what
adjust interval based on a measured SCr obtained 6-14 hours after dose
amino glycoside limitations
- achieve fixed peak and trough
- can’t individualize
- based on average not unique to patients
(pregnant will have a lot of fluid and peak will be higher than it should be, cause toxicity)
aminoglycoside nomogram is used for
tobra and gent, amikacin has a different nomogram
what if the dot is on the border
go up a dose
what is the time on the nomogram
time from start of infusion and time from sample draw
look at two things for 1/ daily amino therapy
- good crcl
2. pt is obese or not
