Occlusal factors in failure Flashcards
(20 cards)
What are some clinical signs that suggest occlusal failure? ๐ค
๐ฆท Fractured teeth or restorations
๐ฌ Increased tooth mobility
๐ฃ Pain on biting
๐ Loosening of crowns
What causes occlusal failure? ๐
โ ๏ธ Excessive forces or forces in the wrong direction (especially lateral forces)
๐ฎโ๐จ Premature contacts
โก๏ธ Lateral forces on posterior teeth
๐ช Strong forces acting on weak or compromised teeth
How can we prevent occlusal-related failures? ๐ก๏ธ
๐ Perform thorough occlusal screening exams on every patient
Check:
๐ ICP (Intercuspal Position)
โ๏ธ Lateral guidance
๐ฌ Repeated restoration failures
๐ Protrusion forces on restored anteriors
๐ฎโ๐จ Wear facets on at-risk teeth
๐ RCPโICP slide in anterior wear
โก Non-working side interferences (especially if palatal fractures are seen!)
What is the role of canine guidance? ๐
๐ฅ Protects posterior teeth by taking lateral load
๐งฑ BUT! Canine must be structurally strong to do this
โ๏ธ Preferable when possible
What is group function? ๐ฅ
๐ง Seen more commonly with age
โ๏ธ Acceptable when load is shared across multiple posterior teeth
โ๏ธ Cusps should be flatter to reduce damaging forces
What should you avoid when planning lateral guidance? โ
๐ซ Should NEVER be on a post-crowned tooth
โ Avoid placing guidance on a bridge pontic
What is a diagnostic wax-up and when is it used? ๐งผ๐ฆท
โ๏ธ A mock-up of the teeth using wax
โ
Mounted in RCP (Retruded Contact Position)
๐ง Done on an articulator
๐ Helps visualize occlusal possibilities and changes
What is an occlusal interference? ๐ง
๐ฌ A heavy tooth contact that disrupts the normal path of jaw movement
Can occur during:
๐ ICP
๐ RCP
โฉ Protrusion
๐ Working-side
๐ Non-working side
What are the effects of ICP interferences? ๐ค
๐ May feel โhighโ to the patient
๐ฆท Causes:
Pain
Increased mobility
Tooth sensitivity
Restoration failure
โ ALWAYS correct occlusion before the patient leaves!
What are working side interferences? ๐คจ
๐ฃ Occur when canine guidance is missing
โ Lateral forces affect:
๐ฆท Buccal cusps of upper teeth
๐ฆท Lingual cusps of lower teeth
๐ก BULL Rule = Buccal Upper, Lingual Lower
What are non-working side interferences and how do they present? โ
Happen when teeth on the opposite side contact during lateral movement
๐ฆท Affects:
Palatal cusps of upper teeth
Buccal cusps of lower teeth
๐ก PUBL Rule = Palatal Upper, Buccal Lower
๐ Wear pattern: Semi-circular on upper teeth
๐ฎ Can lead to mobility and fractures, especially palatal
What occlusal factors should be considered when planning restorations? ๐ ๏ธ
๐งพ Include occlusal โcheck-upโ in every exam
๐ Is ICP stable?
๐ฆท Are teeth in guidance structurally compromised?
๐ง Match occlusion before and after treatment
๐งฐ Choose articulator based on lateral force risks:
๐งโโ๏ธ Conforming: Use ICP record
๐ Re-organizing: Use RCP
Case 1: LL6 has lingual cusp fracture + cracked tooth syndrome. Interference? ๐ค
โก๏ธ Working side interference
๐ก Solution: Remove interference + proper restoration
Case 2: 54 y/o female, pain on biting UL7 (palatal cusp mobile). Interference? ๐
โก๏ธ Non-working side interference
๐ก๏ธ Needs an occlusal splint to protect against bruxism (grinding)
What happens during non-working side interferences? ๐ณ
Teeth on the opposite side of the jaw clash unexpectedly during movement ๐ฑ
Case 3 - 67yo getting crowns, says bite feels โhigh.โ Whatโs the issue? ๐๏ธ
โก๏ธ ICP interference
๐ฉโ๐ง Solution:
High in excursion? โ๏ธ Reduce cusp height
โFreeโ in excursion? ๐ Deepen the fossa
Case 4 - 36yo male keeps breaking anterior composites. Why? ๐ฉ
A23:
โก๏ธ Protrusive interference
๐ก Build up canines to take the force off the front teeth ๐ช๐ถ
Can you bring back canine guidance if itโs worn down? ๐
Yes! But you must evaluate first:
๐งช Periodontal status
๐ Pulp vitality
๐ธ Periapical radiograph
๐ Root length + angulation
๐ซ No RCTs or posts!
How do you plan to rebuild canine guidance? ๐
๐ฆท Take study models
๐ง Record RCP
๐งฐ Mount on semi-adjustable articulator
๐ Request diagnostic wax-up from the lab
โ๏ธ Plan build-up for canines to protect anterior teeth
What is Shimstock used for? ๐ช
Itโs a thin (8ฮผm) metal foil used to detect occlusal contacts โ if the tooth holds the Shimstock while biting, itโs in contact ๐๐งพ
