Skin
The appearance and condition of the skin, Is another important indicator of the bodies Profusion status. In assessing the skin, You should check color, Temperature, Condition, And capillary refill.
Skin color
Skin color indicates how well the blood is oxidated and circulated to the skin ( Respiratory and circulatory system)and therefore how well the lungs and hearts are functioning. In all patients, check the color of the nail beds, oral
Mucosa(mucous Membrane of the mouth) And conjunctivia( Mucous membranes that line the eyelids)
Abnormal skin color includes the following
paleness or pallor
This may be a sign of extreme vascoconstriction, Blood loss, Or both.
Blue gray color or cyanosis
This indicates inadequate oxygenation or poor perfusion.
Is red color or flushing
This may Be a sign of heat exposure, Peripheral vasodialation, Or a very late finding in carbon monoxide poisoning.
Yellow color or jaundice
This may indicate liver disease
Mottling
This is a discoloration similar to cyanosis, However, It occurs as a blotchy pattern. Mottled Skin.
May be seen in some shark patients or patients with blood pooling in the extremities for a long period of time.
Skin temperature
Hot
Which indicates a fever or exposure to heat
Cool
Which may be a sign of Inadequa.
Te circulation shock or exposure to cold
Cold
Which indicates extreme exposure to cold
Clammy skin
Wet and moist
Mate indicate shock poisoning a heat-related.
Cardiac or diabetic emergency.
Or many other conditions.
Diaphoresis
Is the term used to describe profuse sweating.
Extremely dry skin
Maybe a sign of spinal injury or severe dehydration
Capillary refill
The time it take For Compressed capIllaries to fill up again with blood. It is a more reliable sign in infant and younger children than in older children or adults because younger children usually have very little underlying diseases that might affect the profusion of the capillaries.
Normal capillary refill signs
When assessing at room temperature, the upper limits of normal vocabulary Rayfield times are 2 seconds for infants.Children and males adult three seconds for females and four seconds for elderly.
Pupils
Size

A brightly lit Environment.
Equality
PupilS of unequal size may indicate a stroke,, Head.
Injury, an artificial eye, Disease of the eye, Use of certain eye drops, Or injury to the eye or nerve that controls the pupil.
Reactivity
Refers to the pupil changing in size in response to light shining in the eye. The pupils which are normally midsize,constrict when light is shined in them.the pupils dilate when shaded or in a dark environment.
