what is Phacolytic glaucoma ?
Leakage of high molecular weight lens proteins from hyper mature cataract to AC then Macrophage will phagocyted it and cause blockage
Symptoms of Phacolytic Glaucoma ?
Sudden onset of:
Severe ocular pain
Redness
Blurred vision
Halos around lights
Headache or nausea (from acute IOP rise)
Signs of Phacolytic Glaucoma ?
Very high IOP (often >40 mmHg)
Corneal edema
Deep anterior chamber
White, milky aqueous (proteinaceous material)
Floating macrophages (seen on slit lamp as “flare with particles”)
No synechiae or keratic precipitates
Mature cataract with poor red reflex
Open angle on gonioscopy
Treatment of Phacolytic glaucoma ?
1.Initial IOP control
- Topical IOP Lowering Agents
- Diamox
- Hyperosmotic
- Avoid mitotics
2.Get rid of lens
> cataract extraction
3. Post operative care
