Pressure Sore
Basic body positions (7)
Prone, supine, fowler, semi-fowler’s, side-lying, sims, trendelenburg position
For asthmatic patients
Orthopneic position
Static shortening of muscle and connective tissue that limits range of motion at a joint
Contracture
Partial or complete removal of a limb; may be congenital or acquired (traumatic or surgical)
Amputation
Contracture, sitting position
Hip and knee flexors; hip adductors and internal rotators; shoulder adductors, extensors and internal rotators
Preventive position for Transfemoral Amputation
Avoid: prolonged hip flexion and abduction
Encourage: prone position
Transtibial amputation, preventive position
Avoid: prolonged hip and knee flexion
Recommend: lying in prone position periodically
Preventive position, hemiplegia
Upper Extremities: avoid prolonged shoulder abduction and internal rotation; elbow flexion; Forearm supination/pronation; wrist, finger, or thumb flexion; finger and thumb adduction
Lower Extremities: avoid prolonged hip and knee flexion; hip external rotation and ankle plantar flexion and inversion
Involvement of musculoskeletal system (joints)
Rheumatoid Arthritis
Preventive positioning, rheumatoid arthritis
Avoid: prolonged immobilization especially in flexion position
Recommend: protect immobile person bony prominences.
Avoid, recommend, and general rule for split-thickness and grafted areas
Avoid: positions of comfort for extended period; prolonged flexion or adduction or most peripheral joints
Recommend: passive to active exercise
General rule: prolonged immobilization and failure to change position frequently will lead to contracture, pressure sore and decreased functions
Orthopedic surgical condition, keep affected knee in extension
Total knee replacement
Orthopedic surgical conditions, avoid excessive hip adduction, rotation and flexion (beyond 90 degrees)
Total hip replacement
