What is considered postpartum hemorrhage after a vaginal birth?
Blood loss > 500 mL
What is considered postpartum hemorrhage after a C-section?
Blood loss > 1000 mL in 24 hours
What does a boggy fundus indicate?
Uterine atony (lack of contraction)
What are some assessment findings of postpartum hemorrhage?
Boggy fundus, pallor, cool skin, dizziness, hypotension, tachycardia
What does it mean if the fundus is above the umbilicus?
The uterus is not contracting effectively
What is the first intervention for postpartum hemorrhage?
Massage the fundus
Where should the fundus be located within 24 hours postpartum?
Firm, midline at the umbilicus
How much does the fundus descend daily?
About 1 fingerbreadth per day
What should you do if the fundus is not midline?
Have the patient empty their bladder
Why do you weigh peripads?
To monitor blood loss accurately
How much bleeding is too much postpartum?
Soaking more than 1 peripad per hour
Why is oxygen administered in postpartum hemorrhage?
To improve oxygenation and tissue perfusion
What fluids are given for volume replacement?
0.9% Normal Saline or Lactated Ringers
What labs and prep are needed for blood transfusion?
Type & cross, hemoglobin, hematocrit
What kind of IV is needed for postpartum hemorrhage?
Large bore IV catheter
What medications help the uterus contract?
Misoprostol and Oxytocin
What are additional uterotonics used in postpartum hemorrhage?
Methergine and Hemabate
What does Methergine do?
Contracts the uterus to reduce bleeding
What does Hemabate do?
Stimulates uterine contractions to control bleeding
