Management of PPROM?

A

Admitted and expectant management until 37 weeks including for signs of chorioamniotnitis

Medical: erythromycin for 10 days until in labour

Betamethasone/dexamethasone for 24 hours - to accelerate foetal lung maturation if 34 weeks or under

Magnesium sulphate - to support brain development in fetuses 30 weeks or less and in labour OR if planned labour within 24 hours

What should you never offer to someone with PPROM and why?

A

Tocolysis - increases risk of chorioamnionitis as it prolonges labour

A

Foetus:
Pre-term labout
Cord Prolapse

Mother:
Chorioamnionitis -> sepsis
Endometritis -> sepsis
Post partum haemorrhage
