A spike in the female partner’s temperature indicates that ovulation has occurred
The couple then abstains from sex for three additional days.
What are the cons of NFP? (2)
1) requires commitment from both partners
2) failure rate is 25%
Who may NFP be ideal for? (2)
1) Dedicated couple → It requires a great deal of commitment from the couple in order to be effective and interferes with spontaneity.
2) Those with religious reasons
What is the first like tx for infertility?
usually something oral → clomiphene citrate (clomid)
what tx is second line for infertility & why?
something injectable → human menopausal gonadotropin (pergonal)
the adverse effects are higher than oral med (clomid)
A hx of pelvic inflammatory disease (PID) or STIs may cause what?
tubal infertility
why are obese people at risk for infertility?
failure to ovulate (can be from PCOS)
what age declines in fertility?
people over 35 y/o
What should be taught in preconception care? (5)
1) Recommended prenatal vitamins
2) Abstinence from alcohol, nicotine, etc
3) Talk about contraception for people who are not planning to be pregnant
4) Review meds that the pt is taking that might pose risk to developing fetus
5) Recommend a daily multivitamin with folic acid
How to know what advice the pt needs in preconception counseling?
Get a health hx including family hx → bc it will tell you a lot of info about their genetics
What is included in preconception care? (6)
1) review of general health
2) family hx
3) menstural hx
4) medications
5) lifestyle factors
6) vaccination status
what may be involved in preconception care? (5)
1) referrals for nutritional support
2) social services
3) genetic counseling
4) medical specialist care
5) smoking cessation
if a pregnant woman’s last menstrual period was March 17, when is her estimated due date?
LMP → March 17th
Go back 3 mos → December
Add 7 days → 24th
EDD → 12/24
What do we want to see on NST?
we want variability & are looking for 2 accelerations of fetal heart rate in 20 min!!
on an NST, if accelerations do not occur within 40 mins, what does this mean?
the baby is not going to & you would refer mother for more testing
what does a positive NST result mean? (2)
1) Reassuring the baby’s nervous system is responding to changes in the uterus & demands for energy
2) Tells us that acid base balance is normal for the fetus
NST is not reliable until ___ weeks gestation

what is biophysical profile and who is it reserved for?
Test that assesses fetal well being → usually reserved for mothers at risk for fetal conditions (not routine for every pregnancy)
a biophysical profile score of 8 or 10/10 means what?
it is reassuring (good)
a biophysical profile score of 6/10 means what?
monitoring is needed; the baby might not be doing well
