Identify old methods of pregnancy confirmation
Palmers sign: 4-8 weeks contractions of uterus (bimanual examination)
Hears sign: softening of isthmus (bimanual examination)
Goodalls sign: 6 weeks cervix softens
Identify new methods of identifying pregnancy
Through pregnancy test (immunological testing), hCG (human chrionic gonadotrophin) a hormone produced during pregnancy the cells surrounding the embryo produce this hormone. They’re hCG attaches to the fixed antibodies on the second line of the urine test indicating pregnancy.
Ultrasonography scan to see gestation can be identified from 4weeks, or transvaginal probe enters vagina.
How to calculate EDD/EDB
If based on 28 day menstrual cycle u add 9 months and 6 days if 27 day cycle u minus 9 from final count if 26 day u minus 18 from EDD .
Assessing gestational age done in weeks and days eg if 10+2 the woman has gestational of 10nweeks and 2 days.
Explain importance of booking appointment
To calculate EDD and conduct a booking appointment before 10 weeks gestation to discuss plans for birth and antenatal care, and forming trust with midwife.
Explain elements of booking histroy
Past medical info, diabetes, cardiovascular, trauma to pelvis, FGM, mental health, or surgical; anaesthetic, type of surgery, blood transfusion.
Obstetric history;previous c section, miscarriages,perineal tears, breech birth.
Ethnicity; as some conditions are genetically linked like; sickle cell, thalassaemia, diabetes. Or cultural eg FGM.
Lifestyle; smoking, alcohol, drug use, (all offered specialist help), lack of exercise
Discuss required skills midwife must facilitate for an effective booking
Ability to recognise family situations and work autonomously, provide assistance whatever the situation may be and always be compassionate and be able remain professional. Be able to carry out basic clinical skills and to be able to risk assess.
