When should you do a test drive with trial frame?
Large change in pt's sphere Change in cyl power or axis First pair of glasses Cyl is prescribed for first time Pt does not have previous glasses and can't determine amount of change Any concerns about prescription
What are some general prescribing cautions?
Avoid increasing subjective symptoms
Avoid large optical changes
Avoid poorly designed lenses and frames
Avoid ineffective communication
What is the goal?
Single clear comfortable binocular vision.
What should each patient be considered in terms of?
Age Degree of symptoms Amount of ametropia VA Binocular function Efficiency during the performance of visual tasks Presence of astigmatism or anisometropia Previous spectacle Rx
A 37 year old with a single vision hyperopic correction in their glasses is seeing well at distance, but is having difficulty reading.
Insufficient amount of old prescription
Do a cycloplegic refraction on them to see how much latent hyperopia is needed. Push the Plus on them
A 6 year old patient is examined and found to have a refractive error of +1.25 in each eye. Should glasses be given?
No Rx necessary. Emmetropization will tend them toward emmetrope
A 50 yr old who was successfully using over-the-counter reading glasses is now beginning to have trouble with distance vision. Why, and what might you recommend?
Latent hyperope turned into absolute hyperope. Possibly give them bifocal
A 37 year old myope seeing well at distance with their glasses is having trouble reading.
Pt is overminused
Educate them and tell them it will take time to adapt
A 55 yr old high myope presents for routine examination. You determine that they do not need a change in glasses and that their eyes are in excellent health. When discussing those results, what else should you tell them?
Retinal detachment, watch for flashes and floaters, immediately come in if it happens.
A 30 year old pt who has never worn glasses is examined and found to have a small amount of myopia. The pt says they do not feel they need distance glasses. Should you prescribe them?
Educate and let them try a trial frame and let them take an active part in it.
A 35 year old wearing glasses for myopia is examined, and you measure a very slight increase in their myopic correction. Should you make it?
It depends, let the pt decide by showing them their prescription and the new prescription. Trial run to see if they want it.
A 55 yr old pt, calls and says “their new glasses you gave them are not good. I can’t see well with them.” What should you do?
