RF - 1mg/kg/12hrs
both of these are until oral anticoag is established

A
apixaban 10mg for 7 days, 5mg 3-6 months
recurrent DVT and PE 2.5mg ongoing after 6 months treatment

A
1L 0.9% saline over 1 hour, 2 hours, 4hours, 8 hours
FRII 50U in 50ml 0.9% saline (infuse at 0.1unit/mg/hr)
give 10% glucose once <14
