what is psoriasis?
inflammatory skin condition usually characterised by well-defined scaly plaques ⁷

presentation?
red scaly plaques that are often symmetrical
what causes psoriasis?
overactive immune system
epidermal infiltration by activated T cells leading to increased epidermal cell proliferation and turnover
3-5 day cycle from 28day
what is there excessive production in?
TH1 cytokines: TNFa
linked to flares
histology in psoriasis? (5)
hyperkeratosis neutrophils in stratum corner hypogranulosis psoriasiform dilated dermal capillaries
what is the most common form of psoriasis?
chronic plaque psoriasis
Guttate psoriasis?
acute onset, post-viral
self-limiting
step infection
responds well to phototherapy
palmo-plantar psoriasis?
skins on palms and soles of feet appear thick, scaly and red
scalp psoriasis?
kyperkeratotic plaques on scalp
nail psoriasis?
pitting and oncholysis
flexural/inverse psoriasis?
shiny and lack of scale in armpits and flexes
pustular psoriasis?
red tender patches with multiple yellow pustules on inspection
erythrodermic psoriasis?
red man syndrome

admit: fluid balance, bloods & thick greasy ointment emollients
differential diagnosis? (3)

lichen planus
mycosis fungoides (biopsy)
