Mini Mental State Examination (Folstein Mini-Mental)
A widely used, screening test for cognitive functioning for individuals with cognitive or psychiatric dysfunction.

Bay Area Functional Performance Evaluation (BAFPE)

Goal Attainment Scaling (GAS)
facilitates active participation in the goal-setting process by having individual and/or caregivers identify desired intervention outcomes for the client that are personally relevant to them
can be used post-intervention to assess attainment toward goals

assesses self-reported role participation and the value of specific roles to the individual
