Is radiographic evidence needed to diagnose OA in people older than 40?

Benefits of x-rays
- Cost effective
- Quickly obtained
- Augment understanding of OA
Positioning for pt during x ray, number of views needed
Needs to be load bearing, at least two views
Differential diagnosis with x ray
Used to rule out red flag causes, such as sepsis, tumour, #, inflammatory conditions
Average exposure compared to when we get x rays
1-3mSV/year. X ray is equal to 3 hours - 10 days of exposure.
MRIs good for OA
Nah, all soft tissue, NWB
Bone scan helpful for detection of OA?
Yep, also Dx osteomyelitis, bone metastases, metabolic bone disease, implant failure
Symmetrical joint space loss indicative of OA?
Nope, probably something inflammatory/systemic

1: doubtful JSN, possible osteophytic lipping
2: definite osteophytes, possible JSN

4: large osteophytes, marked JSN, severe sclerosis, definite bony deformity
Outerbridge Arthroscopy

1: cartilage softening swelling
2: partial thickness defect with fissures, less than 1.5cm in diameter
