CKD means:

  • no 1alpha hydroxylase > no vid D hydroxylation > low vit D > low calcium absorption
  • lower phosphate excretion (as phosphate is normally excreted by kidneys)

The high serum phosphate drags calcium out of bone > osteomalacia
