1
Q
What is the recommended treatment option to slow the progression of autosomal dominant polycystic kidney disease?
A
Tolvaptan (vasopressin receptor 2 antagonist)
2
Q
How does tolvaptan work to slow disease progression in autosomal dominant polycystic kidney disease?
A
- Selective & competitive vasopressin 2 (V2) antagonist
- Binds to the V2 receptors in the renal collecting ducts to reduce cell proliferation, cyst formation and fluid excretion
- This reduces kidney growth & protects renal function
3
Q
Give three examples of conditions that can be treated using vasopressin receptor 2 agonists (e.g. desmopressin).
A
- Diabetes insipidus
- Nocturnal enuresis
- Von Willebrand disease
4
Q
What is the screening investigation for relatives of patients with autosomal dominant polycystic kidney disease?
A
Abdominal ultrasound
5
Q
When does NICE recommend treating autosomal dominant polycystic kidney disease with tolvaptan?
A
- Chronic kidney disease stage 2 or 3 at the start of treatment
- Evidence of rapidly progressing disease
- Discount agreed in patient access scheme
