rhinovirus
Huge white out of one lung field?
Unilateral pleural effusion = malignancy or infection (trachea pushed away)
Lung collapse - trachea pulled towards side of white out
Transudate v exudate pulm eff?
Transudate protein <30g/L
Exudate protein >30g/L
Transudate is systemic disease whereas exudate indicates local disease
Small cell paraneoplastic syndromes?
ADH - low Na
ACTH - cushings
Lambert Eaton syndrome
Squamous cell paraneoplastic syndromes?
PTH-rp causing hypercalcaemia
COPD symptoms in a young person
alpha-1 antitrypsin (A1AT) deficiency
First line for COPD?
SABA or SAMA (ipratropium)
Second line for COPD?
no asthmatic features = LABA + LAMA
asthmatic features = LABA + ICS
Presentation of cor pulmonale?
Breathlessness Peripheral oedema Dizziness chest pain Raised JVP S3 Murmur Hepatomegaly bc of back pressure into hepatic vein
FEV1 and FVC findings?
FEV1 Reduced in obstructive lung
FVC reduced in restrictive lung
What is FVC?
Total amount of air a person can exhale after full inhalation
When is obstructive lung disease diagnosed?
When FEV1 is <75% of FVC
When is restrictive lung disease diagnosed?
FEV1 and FVC are both reduced, FEV1 is >75%
Examples of restrictive lung disease?
Pulm fibrosis Sarcoidosis Obesity Scoliosis of the spine MND

Small cell and non small cell
NSCLC: squamous and adenocarcinoma
