In judging a broad tachycardia, what would favour VT or SVT, respectively?

A

Every broad tachycardia should be considered VT until proven otherwise.
Concordant QRS polarity in all chest leads would favour VT
Morphology that shows a clear bundle branch block favours SVT with abberances
Evidence of P waves fusion beats and capture beats: VT
Same QRS morphology as in SR: SVT

What is a capture beat? How is it recognized?

A

Atrial impulse completely depolarises the ventricle before being depolarised by ventricular mature beat
–> normal beat in the middle of tachycardia

What is a fusion beat?

A

Partial depolarisation from atrium, partial from ventricular premature beat –> normal P wave, abnormal QRS
