BN 408 from E423 here is my on scene report, E423 on scene side A, I have a 2 story single family dwelling with fire showing from floor # __ window ___ quadrant, My crew will be extending a 1 3/4 line to side A to get an exterior knock on the fire while I take my lap. E423 on scene water established I have pulled 1 address past the dispatched address making room for the truck and water is secured. Fairfax from E423 start a RIT task Force, stand by for situation

Lap complete label the floor basement 1, 2, basement is a walk up or walk out basement, basement is clear of smoke and fire with access on side Charlie. Status of the occupants are unknown. Engine 408 take a line below the fire and report back, T408 X-ray your crew and force entry give me a primary search on floor number 1 force , T408 exterior crew ladder side A and C and report. E423 making entry to floor number 1 with a crew a 3 for a transitional attack, request to transfer command.

SITUATION

Lap

Battalion 402 from Engine 423. Lap completed. Two in the front, three to the rear with an exterior
gas meter on side Bravo. Floors are Basement, 1, and 2. Basement is clear with access on side
Charlie. All occupants are out of the structure and report an odor of gas in the kitchen, Floor 1,
quadrant Charlie. Engine 401 has a 1 3/4-in. line staged in front of the Bravo exposure,
investigating with a crew of two. Truck 401 is investigating in the basement with a crew of two,
two remaining outside. Requesting to transfer command

Response for fuel leaks

A

Ascertain type of incident: fire, spill, fire with spill.
 From a distance (500 feet) use binoculars to look for placards or other warnings and the
product name or number on the tractor or trailer.
 Get information from people on the scene (driver, eyewitnesses, shipping papers or other
identifying paperwork).
 Determine scope of incident: number of involved vehicles and/or people affected.
 Determine best scene access.
 Determine water supply.
 Pay extra concern to exposures. Allow the tanker truck to burn and assign units
cool/protect the exposures.
 Remember that the tractor or uninvolved portion of a multi-unit (tractor trailer or multiple
trailers with a single tractor) is an important exposure. It may be necessary to disconnect
the tractor or unhitch a trailer and remove it from exposure. Diking around the vehicle or
damming a drainage ditch may be required to limit a running-spill fire.
 Use master streams or other unmanned heavy caliber streams to keep suppression
resources remote.
 Attack from high ground with the wind at your back, checking for any overhead wires
and coordinating streams so they do not oppose each other. Flush the fire away from
exposures and into less hazardous areas. If agent is limited, use it to protect exposures.
Agent that runs off is wasted. Do not wash away foam blankets with water streams.

Send
 Give a situation report to communications.
 Request additional resources as needed (such as hazardous material, foam).
 Initiate evacuations as needed; request law enforcement to assist.
 Warn any incoming units of hazards.

Set Up
 Establish and maintain command until relieved.
 Declare an operational mode: offensive, defensive, non-intervention.
 Establish a staging area.
 Set up scene access and egress.
 Set up a perimeter using fire line tape or other means.
 Establish an isolated holding area for personnel or victims who become contaminated.
Patient Triage
 Consider both the patients for triage as well as vehicles and any other problems which
may be occurring or have occurred.
 Assign units to triage people or problems – you may have to step back from the incident
and send in a reconnaissance team in appropriate PPE to check the situation. The team

should go in slowly and use combustible gas indicators and explosion proof lights and
radios – and stay out of observed spills.

You are the OIC on E444. You are dispatched as the 1st engine to a reported inside gas leak at a garden apartment.

•Additionally, during operations at this incident, the gas concentration is determined to be 50% LEL.

•List all of the actions you would take

A

Establish Command and Give Size-Up
• Assume Incident Command and provide an initial report:
“E444 on scene of a 3-story garden apartment with a reported inside gas leak. Initiating investigation and establishing Garden Apartment Command.”

  1. Scene Size-Up
    • Identify:
    • Occupancy type (multi-unit)
    • Utilities access (meters, shutoffs)
    • Occupants still inside
    • Access/egress points
    • Wind direction (affects gas dispersal)
  2. Evacuate the Immediate Area
    • Evacuate the affected unit and adjacent units (above, below, beside).
    • Expand evacuation as needed once meter readings are confirmed.
    • Keep occupants upwind and at a safe distance.
  3. Control Ignition Sources
    • Do not operate any electrical switches.
    • Prohibit smoking, radios, flashlights, or other electronics in the hazard zone.
    • Do not use fans or mechanical ventilation.
    • Request the power company to secure electricity if needed.
  4. Investigate with Gas Monitor
    • Enter with full PPE and a calibrated 4-gas meter.
    • Monitor for:
    • % LEL (flammability)
    • Oxygen levels
    • Carbon monoxide
    • Check low areas (gas may sink) and potential accumulation zones.

⚠️ WHEN GAS CONCENTRATION IS AT 50% LEL

🔴 6. Immediately Withdraw All Personnel
• A 50% LEL reading is a dangerous, explosive atmosphere.
• Order an immediate withdrawal from the affected structure.
• Do not re-enter until levels drop well below 10% LEL.

🔴 7. Upgrade the Incident
• Request:
• Hazmat Team
• Gas utility company
• Battalion Chief
• Additional suppression or EMS units if needed
• Notify all units on-scene of hazardous conditions.

🔴 8. Establish Hot, Warm, and Cold Zones
• Hot Zone: Interior and immediate exterior of affected unit
• Warm Zone: Staging/entry control, PPE required
• Cold Zone: Command post, public/occupant holding area

🔴 9. Shut Off Gas Supply (If Safe)
• Attempt to isolate the individual unit’s gas supply if the shutoff is external and accessible.
• If unsafe, wait for the gas utility to arrive and secure the system.

🟡 POST-WITHDRAWAL ACTIONS

  1. Ventilate Naturally (ONLY If Safe)
    • Open windows and doors from a safe location and only with Command approval.
    • Avoid powered fans or equipment that could cause sparks.
    • Let the gas dissipate and monitor atmosphere continuously.
  2. Continue Atmospheric Monitoring
    • Periodically recheck readings inside and outside the affected areas.
    • Re-entry is only permitted when readings are consistently below 10% LEL and confirmed by Hazmat or command staff.

PROCEDURES
A. Reporting job-related injuries and/or illnesses that DO NOT require medical treatment
1. Responsibilities of employee shall include:
a. Immediately report all job-related injuries and/or illnesses to their immediate
supervisor.
b. If the employee decides that medical treatment is required, they shall follow
the procedures outlined in this Standard Operating Procedure (SOP)
Section II. PROCEDURES B.1.
2. Responsibilities of supervisors shall include:
a. Immediately notify the on-duty battalion chief, uniformed fire officer, and Safety
Officer (SAFO) of the injury and/or illness.
b. Ensure full legal name of employee and location, where the injury and/or illness
occurred, are recorded in the work location’s logbook.
 If the injury and/or illness occurs during an emergency incident, it also
shall be documented on the appropriate incident report. Example: A
firefighter experienced an injury while conducting a primary search on
this incident.
 Specific medical diagnosis shall not be recorded in the logbook or
incident report.
 The supervisor shall be responsible for reporting all injuries and/or
illnesses if the employee is unable to accomplish this task.
c. Assist in the investigation of the circumstances of the injury and/or illness.
d Complete the Employer’s Accident Report online (using Documents/Safety
Forms on department’s Intranet). After submission of the form, a copy shall
be printed (showing the confirmation number at the top).
e. Complete the Supervisor’s Report of Work Related Injury and/or Illness form

Ensure a completed Employer’s Accident Report and FRD-071 are forwarded to
the battalion chief as soon as possible but prior to the end of the scheduled
workday.
g. For injured and/or ill volunteers, supervisors shall ensure the following is
completed:
H. Ensure that the Volunteer Firemen’s Insurance Services (VFIS)
Accident/Sickness Claim Report is completed and forwarded as part of
the injury packet.
I. Leave a voice mail message with the volunteer liaison indicating who
was injured and whether medical treatment was sought or not.
J. The Volunteer Chief should also be notified.

Medical injury requiring transport

A

Reporting job-related injuries and/or illnesses for employees who require medical treatment
1. Responsibilities of the employee shall include:
a. Immediately report all job-related injuries and/or illnesses incurred to their
immediate supervisor

b. c. d. e. f. g. h. i. Obtain initial treatment at an appropriate medical facility (emergency room
[ER] or urgent care). Employees being transported for an injury and/or illness
shall be taken to the closest appropriate ER.
Complete the Injury and/or Accident Narrative Supplement form (FRD-317) if
the supervisor requires a written statement.
Report work status to their immediate supervisor and forward a completed
Medical Status Report form (FRD-074) to the duty SAFO. If recommended for
modified duty, the employee shall contact the RTWPM for instructions on light
duty assignment.
Continued or follow-up medical treatment shall only be obtained from a
physician on the County of Fairfax Authorized Physicians’ Panel for Workers’
Compensation list unless referred by Fairfax County’s claim service company to
a physician whose specialty is not on the approved list.
 Without this referral, any treatment by a physician who is not on the
approved list shall become the employee’s responsibility for payment.
Exception: Any dentist may be used for treatment of dental
injuries incurred while on duty.
The treating physician shall complete FRD-074 for each visit. It shall be the
employee’s responsibility to provide medical status updates from the treating
physician to the RTWPM if the next appointment is not clearly indicated on
FRD-074. All medical status, Therapy Report form (FRD-091), and work status
report will be sent to the RTWPM within 24 hours of the appointment
and should clearly indicate the return to work status. Follow the treatment orders
of the attending physician at all times.
Notify the treating physician or medical treatment facility that all reports and
bills shall be sent to Fairfax County claims service company.
Notify the RMD if a claim acknowledgment letter is not received from
Fairfax County claims service company within 15 days after reporting the
injury.

Responsibilities of supervisors shall include:
a. Complete all requirements in Section II. PROCEDURES A. 2. of this SOP.
b. Ensure that initial medical treatment is provided to the employee.
c. Provide the injured employee with a County of Fairfax Authorized
Physicians’ Panel for Workers’ Compensation list and FRD-07
