Characterized by a broad spectrum of cognitive and emotional dysfunctions including delusions and hallucinations, disorganized speech and behavior, and inappropriate emotions.
Schizophrenia
Complete recovery is normal in schizophrenia
No. It is rare.
Emil Kraepelin
Catatonia - Altering immobility and agitation.
Hebephrenia - imature emotionality.
Paranoia - delusions of grandeur.
Positive Symptoms
Symptoms around distorted reality
Negative Symptoms
Absence or insufficiency of normal behavior
Disorganized Symptoms
Rambling speech, erratic behavior and inappropriate affect
Diagnosis of schizophrenia
- Present at least one month
- two or more positive, negative and or disorganized symptoms
- At least one includes delusions, hallucinations or disorganized speech
- Symptoms should persist for 6 months
Types of Positive Symptoms
Delusions
Hallucinations (auditory is most common)
Cotard vs Capgras
- I am dead
- Someone I know has been replaced by a double
Avolition
- Show little interest in performing even basic day-to-day functions
Alogia
- Brief Replies
Anhedonia and Affective Flattening
- Pleasure
- Emotions
Disorganized Speech
1.Tangentiality - going of on a tangent instead of answering a question
2.Loss of association or Derailment - abruptly changed the topic
Wavy Flexibility
Keep their bodies and limb in the position they are out in by someone else
Catatonia
Motor dysfunctions that range from agitation to immobility
Three Division have been historically identified:
Paranoid, Disorganized and Catatonic.
Dropped in the DSM-5
Delusional Disorder
- one or more delusions for at least one month
- Tend not to have flat affect, anhedonia or other negative symptoms
Erotomaniac, Grandiose, Jealous, Persecutory, Somatic, Mixed, Unspecified
- One is loved by another person usually of higher status
- Inflated worth, power, knowledge, identity or special relationship to deity or famous person
- Believes sexual partner is unfaithful
- Believing that I am being treated malevolently
- Feels afflicted by a physical defect or condition
Can hallucinations be found in a delusional disorder?
Yes. As long as it is not dominant.
Substance induced and Associated with medical conditions
Has a delusion or hallucination with laboratory proof that it (the negative symptom ) occurred after substance intoxication OR you have a medical condition.
Schizoaffective
Schizophrenia with Mood Disorders though it can eventually be changed to to either a mood or schizophrenia disorder.
1. Presence of one mood disorder concurrent with Criteria A of schizophrenia
2. Delusions or hallucinations for 2 or more weeks in the absence of a major mood episode
Schizophreniform
Some experience symptoms of schizophrenia for a few months only.
When can we say that Schizophreniform has a good prognosis?
Two of the following features:
1. Confusion or perplexity
2.. Onset of prominent psychotic symptoms within 4 weeks of the first noticeable changes in usual behavior or functioning
3. good pro-morbid social and occupational functioning
4. Absence of blunted or flat affect
Brief Psychotic Disorder
Presence of one or more positive symptoms lasting 1 month or less
Often precipitated by extremely stressful situations.
