A
Lack of meaningful stimuli, distress level out of proportion to the intensity level of stimulus.
Ex: impaired vision (clean glasses), impaired hearing (hearing aids, caption on video, ear wax assess)

Protect pt from injury from seizures before seizure happens
A
- Explain why you are putting the precaution paddings, suctioning supply, airways, closest room near nurses station, make sure pt is getting adequate nutrition and sleep good (sleep deprivation increase risk of seizure)

- Acute/reversible state of confusion, caused by meds, infections, hypoxia (low O2)
- Chronic progressive deterioration of mental status, caused by physical changes in the brain
- Reorient frequently, communicate clearly and slowly, limit choices, promote feelings of security, use pet therapy.
