Define Sepsis
Sepsis is life-threatening organ dysfunction caused by a dysregulated host response to infection
Define Systemic Inflammatory Response Syndrome (SIRS)
SIRS is a non-specific inflammatory response triggered by infection, trauma, burns, or other insults, resulting in dysregulated immune activation
Define Septic shock
Septic shock is defined as sepsis with hypotension, despite adequate fluid resuscitation, resulting in organ hypoperfusion
2 criterias
Septic shock criteria
- Persistent hypotension with MAP below 65 mmHg, requiring vasopressor, despite adq. fluid resuscitation
- serum lactate >2 mmol/L
Sepsis Risk factors
usu. undeerlying abscess / infection
- Age >75 or < 1
- Surgery / procedures
- Indwelling lines / catheter
- DM
- Imm. suppressed
- Meds - chemo, steroids
- IV drug misuse
- Breach of skin - burns
with underlying infection / abscess
Sepsis criteria
- Presence of a known or suspected infection
- Clinical features of organ dysfunction
Sepsis RED FLAGs
- SBP < 90 or drop >40
- RR >25
- HR > 130
- O2 < 92%
- UO < 0.5ml/kg/hr
- Lactate > 2 mmol/L
- Recent chemo
3 in / 3 out
Sepsis 6
- IV Fluids
- High flow O2
- IV Abx
- UO - Catheter
- Blood cultures
- Lactate
National Early Warning Score (NEWS2) parameters
Temperature
Heart rate
Respiratory rate
Oxygen saturation
Blood pressure
Consciousness level
When not to use NEWS2
- Aged under 16
- Inpatients who are pregnant
- Palliative care patients as part of an MDT discussion
what NEWS score indicate sepsis
4 or more
Identification of the infection source Ix
- Urine dip +/- culture
- Chest X-ray (CXR)
- Swabs (e.g. surgical wounds)
- Operative site assessment (via CT or US imaging)
- Cerebrospinal fluid sample (via LP)
- Stool culture
