A
condition characterized by the presence of multiple serrated polyps in the colon and rectum, and is associated with an increased risk of colorectal cancer (CRC).

Diagnosis

A diagnosis of SPS is made if any of the following criteria are met:
- At least 5serrated polyps proximal to the rectum, all >5 mm in size, with at least two >10 mm.
- More than 20 serrated polyps of any size, distributed throughout the colon, with at least 5 proximal to the rectum.
- A first-degree relative diagnosed with SPS and at least one serrated polyp >5 mm proximal to the rectum.

Management
A
- Surveillance:
Colonoscopy every 1–3 years, depending on polyp burden.
Goal: detect and remove polyps early to prevent colorectal cancer.
- Polypectomy:
Removal of all polyps ≥5 mm.
Consider piecemeal EMR for larger lesions.
- Family Screening:
First-degree relatives should have screening colonoscopy starting at age 40 or 10 years younger than the earliest case in the family, repeated every 5 years if normal.
- Surgery:
Consider subtotal colectomy with ileorectal anastomosis i
