In what surgical field does shared airway surgery most commonly occur?
ENT, maxillofacial and neurology.
Why does shared airway surgery carry risk?
Increased risk the airway patency may be compromised.
In what ways may the airway patency be compromised in shared airway surgery?
- Dislodging or damaging the airway device (especially with drapes).
- Blood/mucus compromising the airway and increasing risk of aspiration.
– Surgeons’ use of sharp instruments, all which could damage the airway device.
What is a microlaryngoscopy tube (MLT)?
A paediatric-sized standard ETT with an adult length.
What sizes does an MLT come in?
4.0, 5.0 (most common), 6.0.
What is the purpose of an MLT’s small diameter?
Allows surgeon easier view and access to the vocal cords and adjacent structures.
For what surgery is a north-facing tube used?
Neck, mouth or lower jaw surgery.
For what surgery is a south-facing tube used?
Head, upper jaw and face surgery.
What is an alternative to intubation in shared airway surgery?
Jet ventilation.
What is jet ventilation?
High flow of oxygen being delivered under pressure directly into the patients’ lungs via a catheter.
How does expiration occur in jet ventilation?
Passively, as chest falls.
Why is jet ventilation dangerous?
Oxygen could be forced into the lungs at too high a pressure, causing damage.
What is a Monsoon?
Mechanical jet ventilation.
