Neer sign
Sulcus sign
Used to assess anterior GHJ dislocation/subluxation.
Patient in standing or sitting. Perform shoulder rotation and relax the patient, then allow the shoulder to fall into depression.
A simple beneath acromion indicates a positive test.
Apprehension test
Used to assess anterior shoulder instability and SLAP lesion.
Patient is positioned in supine with shoulder supported on plinth, the elbow flexed to 90 and abduct shoulder to 90.
The examiner holds patient wrist and gently places hand anteriorly on shoulder then externally rotate the shoulder slowly.
Positive if patient demonstrate apprehension during movement, at that point the test is positive.
Negative test indicate subtle instability with secondary rotator cuff impingement.
Relocation test
Only perform when apprehension is positive.
Patient in supine with shoulder supported on plinth.
Examiner applies a firm pressure posteriorly on head of humerus, and continue with rotation.
Positive if ROM increases before patient become apprehensive
