- Mid-face skin ptosis

Non-ideal Patient:
A
- Excessive skin in mandibular region associated with thick skin.
- Excessive skin due to advanced ageing and thin skin.
- Excessive skin in mandibular region but with hypertrophic type ageing.
- Excessively thin skin with lack of fat tissue.

What is the golden rule:
A
The length between the two exit points has to be more than:
8 cone - 10cm
12 cone - 16cm
16 cone - 20cm

Post-operative care:
A
- Antibiotic cream bd for 5/7
- Apply ice/cold towel immediately after procedure for 5 - 10min
- Anti-inflammatory tablets prn (2 - 3/7)
- Head elevated for 3/7 and sleeping face up x 5/7
- Wash and dry face gently without rubbing x 5/7
- Make up, shave, wash hair gently x 5/7
- Avoid direct sunlight / sunbed x 2/52
- Avoid facial massages/aesthetic treatments x 3/52
- Avoid sports x 3/52
- No saunas x 3/52
- No dental surgery x 3/52
- No excessive facial movements x 3/52

Discuss the 12-point process:
A
- Interview with patient with informed consent.
- Prepare the room and material
- Determine zones and vectors - draw with ruler
- Lay down patient and cover with sterile field
- Remove material from pouches
- Administer local anaesthesia at entry and exit points
- Create entry hole with 18G needle
- Insert suture needle vertically through the entry point to depth of 5 mm until the black line disappears
- Manipulate the adipose tissue with fingertips by exercising a soft pressure.
- Trim the free ends of the thread
- Clean the treated area, apply antiseptic ointment
- Provide post treatment recommendations
