What is the TNMG system?
A system for staging soft tissue tumors of malignancy T = size and extension of tumor N = involvement of lymph notes M = Metastasis G = Grade of the sarcoma
What good are CT for soft tissue mass?
Determine size
Extent
And bony involvement
Evidence of malignancy on MRI
Heterogenicit signal intensity, bone or neurovascular involvemednt, ill defined margins.
What imaging should be obtained once malignancy is diagnosed?
Chest X-rays
If mass is found on chest X-ray entire chest CT should be performed.
Tubular globulous appearing of high T2 signal intensity with fibrous tissue of low signal intensity
Hemangioma
How large of margins are generally required for patients with malignant subcutaneous lesions?
3-5 cm margin down to the deep fascaie
What cell origin do neurofibromas come from?
Spindle cells!
What disese are neurofibromas relateed to?
Von Reklenhausen syndrome
Neuroilemoma
Benign tumor arising from peripheral nerve sheath
Look for nodules that cause sensory pain when palpated.
Villous projections of the synovial membrane composed of fibroblasts and giant cells
Pigmented villonodular synovitis
Three different forms
Intraarticular, tenosynovial giant cell tumors, solitary intraarticular tumors
Angioleiomyoma
Tumor arising from the soft tissue of vasculature.
Most commonly diagnosed malignant soft tissue tumor?
Malignant fibrous histiocytoma
Most commonly found malignant soft tissue mass found within the foot?
Synovial sarcoma
Fibrosarcoma
Rare, must be differentiated from the more common Fibroma.
Second most common soft tissue sarcoma in adults?
Liposarcoma
