Q
What is the first line management of stable angina?
A
Secondary prevention: aspirin 75mg, Statin 80mg, and GTN
Symptomatic management:
1st line: Beta blocker (bisoprolol) OR CCB (verapamil/diltiazem). If BB not working, add or switch to a long acting CCB (amlodipine/modified release nifedipine). If initial CCB not working, swap to a BB.
If both CCB and BB not tolerated, switch to long acting nitrate (ISMN), ivabradine, nicorandil, or ranolazine
2nd line: BB + long acting CCB (amlodopine/nifedipine)
3rd line: same as 2nd line + long acting nitrate (ISMN)
4th line: PCI/CABG
