(52 cards)
What study would you use to evaluate typical antipsychotics?
Adams et al (2012)
What did Adams et al (2012) do?
Cochrane review of 50 high quality randomly controlled trials found that chlorpromazine was more effective than a placebo
For what percentage of patients are typical antipsychotics effective?
60%
What percentage of typical antipsychotics users suffer from tardive dyskinesia as a side effect?
20%
50% of typical antipsychotics users suffer from what side effects?
Weight gain and sexual dysfunction
What study would you use to evaluate atypical antipsychotics?
Essali et al (2009)
What did Essali et al (2009) do?
Cochrane review of RCTs found that clozapine led to fewer relapses and greater improvement in functioning and negative symptoms than typicals, and 34% of those who were not improved by typicals were improved by clozapine
What is the main reason why atypicals are more appropriate than typicals?
They have fewer side effects
What is ECT used for?
Catatonic, suicidal or pregnant patients
What 2 studies would you use to evaluate ECT?
Tharyan and Adams (2009)
Greenhalgh et al (2005)
What did Tharyan and Adams (2009) do?
Cochrane review of 26 RCTs found that ECT led to fewer relapses in the short term but not the long term
What did Greenhalgh et al (2005) find?
No evidence supporting the use of ECT on catatonic patients
What 3 issues undermine the appropriateness of ECT?
short term memory loss, historic breaking of bones (procedure has now improved to stop this) and the issue of consent
What proportion of patients are resistant to all drugs?
1/4
What did Cheniaux (2009) find?
34% diagnosis rate using ICD, >20% diagnosis rate with DCM
What did Mojtabi and Nicholson (1995) find?
0.4 inter-rater reliability in whether or not a hallucination was bizarre - suggests open to interpretation
What study would you use to back up Kleitman’s threat of procedure?
Cheniaux (2009)
What study would you use to back up Kleitman’s threat of clinician?
Mojtabi and Nicholson (1995)
What percentage agreement did Beck find between doctors in 1957?
54%
What study would you use to show poor historic reliability of diagnosis?
Beck (1957)
What study would you use to show improvements in reliability of diagnosis?
DSM III tests in the 1980s
What did tests of DSM III in the 1980s find?
81% consistency
What did they do to DSM to improve reliability?
Removed vague definitions and clarified number of symptoms needed for diagnosis
What 3 studies would you use to evaluate family intervention?
NICE, Leff et al (1982), Pharoah et al (2012)
