Not at high CVD risk
* Metformin

If HbA1c still uncontrolled, add either:
* SGLT2i
* DDP4i
* Sulfonylurea
* Piolitazone

High CVD risk

  • Life-threatenin DKA!
    Monitor ketones if treatment is interrupted (surgery/ illness)
  • Fournier’s gangrene
  • Risk of lower-limb amputation (Canagliflozin)
  • UTIs
