The Hip Flashcards
(22 cards)
What is the bursa of the hip?
The synovial membrane that extends as a pockets or sac
What is the purpose of the bursa?
It contains synovial fluid that creates a cushion that prevents structures from rubbing against each other
What are the symptoms of trochanteric bursitis?
Lateral or buttock pain
Pain is worse at night and lying on the affected hip or getting up from a chair
Also worse with prolonged walking, stair climbing
How can you assess trochanteric bursitis?
Palpatine the gluteus medium tendon and it’s painful
It pt resists active abduction or active internal rotation (if there’s pain over the greater trochanter)
What is the labrum?
A ring of rubbery fibrocartilage around the rim of the acetabulum and acts as the suction seal of the hip joint
How would you know someone has a labral tear?
Pain is worsened with activity and stair climbing or sitting
There’s clicking or locking or even giving way
Anterior hip or groin pain
What objective test could be done for a labral tear?
Observe gait(trendenlenburg gait or a leg discrepancy and if therss decreased weight bearing)or posture (leaning on unaffected limb)
Squatting
Passive ROM
What special tests are done to test for labral tears?
Faber
Hip quadrant with longitudinal cephalad
Scour test with clicking
Active SLR to +/- 60 deg (with active DF and then hip flexion alleviates pain)
Thomas Test (There’s clicking and pain when lowering leg and contralateral leg can’t be fully straight)
What are biomechanics of the hip?
Fle/Ext in saggital plane
Abd/Add in frontal plane
Med/Lat rot in transverse plane
What are the two angulation of the femur?
Angle of torsion and inclination
What is the angle of torsion?
Occurs in the transverse plane between axis through femoral head and neck and through the distal femoral condyles
What is the angle of inclination?
Occurs in the frontal plane between axis through femoral head and neck and longitudinal axis of the femoral shaft
What is the normal angle of the inclination of the femur ?
120-135
What is the difference between coxa vara and valga?
Valga is greater than 125 and vara is lesser than 125 degrees
What is the normal angle of torsion?
12-14
What is the difference between anteversion and retroversion?
Anteversion is the increase in angle of torsion(Toeing in)
retroversion decrease in angle of torsion (toeing out)
What could cause limitations in movement of hip?
Perthes disease
TB
Congenital Hip dislocation
Bursitis
Symptoms referred from Lx and SIJ
Trauma
OA
RA
What is the difference between cam deformity and pincer deformity?
Pincer deformity is extra bone extending from edge of the acetabulum and covers the femur and Cam deformity is a Deformity in the shape of the femoral head
What are the symptoms of femoral acetabulum impingement?
Moderate hip or groin pain
Stiffness
Clicking or catching
Pain in thigh,butt,back
Locking or giving way
What special tests can be done for FAI?
Scour
Faber
Faddir
What tests can be done for the ITB?
Ober and Noble test
What functional tests can be done?
Walking
Staur climbing
Running
Hop and standing on one leg
Squatting
