management of post-tonsillectomy haemorrhage in first 6-8hrs
immediate return to theatre
(primary/reactionary haemorrhage)
management of post-tonsillectomy haemorrhage between 5 and 10 days post surgery
admission + antibiotics
–> most assoc with wound infection
criteria for tonsillectomy
7 episodes per year for 1 year
5 per year for 2 yrss
or 3 per year for 3years
+ episodes are disabling, prevent normal functioning
other conditions that might require tonsillectomy
recurrent febrile convulsions 2nd to eps of tonsilitis
obstructive sleep apnoea, stridor or dysphage 2nd to large tonssils
peritonsillar abscess thats unresponsive to tx
Abx mx of tonsilitis if penicillin alergy
clarithromycin
(usually phenoxymethylpenicillin)
FeverPAIN score
FEver >38
Purulence - exudate
Attended rapidly (<=3days)
Inflamed tonsils (severe)
No cough
–> 1 point for each, maach score of 5
feverPain score required for antibioticss
0-1 - low - no abx
2-3 - mod likely - delayed abx (3days)
4-5 - abx tx
