vertigo Flashcards
(15 cards)
examples for vestibular rehabilitation exercises
Brandt-Daroff
type of nystagmus in BPPV
rotatory nystagmus
medication offered in BPPV
betahistine
- of limited value tbh
Menieres disease
disorder of inner ear of unknow cause
- characterised by excessive pressure + progressive dilation of the endolymphatic system
features of menieres
recurrent episodes of vertigo
tinnitus + hearing loss
aural fullness
nystagmus, positive romberg test
menieres management
ENT to cofirm dx
DVLA !!!
acute attack - buccal/IM prochlorperazine
prevention - betahistine + vestibular rehab exercises
management of acute menieres attacks vs prevention of attacks
acute attack - buccal/IM prochlorperazine
prevention - betahistine + vestibular rehab exercises
viral labrynthitis vs vestibular neuronitis
recent viral infection
Labrythitis - Loss of hearing
vestibular Neuronitis - No loss of hearing
vertebrobasilar ischaemia
elderly patient
dizziness on extension of neck
which reflex is commonly lost in acoustic neuromas
absent corneal reflex
V = afferent
VII = efferent
vestibular neuronitis features
recurrent vertigo attakcs lasting hours-days
N+V
horizontal nystagmus - labrynthitis also has
NO hearing loss or tinnitus
what examination is used to differentiate between vestibular neuronitis + posterior circulation stroke
HiNTs exam
- Head Impulse, Nystagmus, Test of Scew
negative = peripheral cause = vestibular neuronitis
positive = central cause - stroke
management of vestibular neuronitis
severe cases = buccal/IM prochlorperazine
less severe = short oral course of prochlorperazine/antihistamine may be used
chronic sx = vestibular rehab exercises
vestibular schwannoma investigation of choice
MRI of cerebellopontine angle
cranial nerves involved in acoustic neuromas + how damage to them presents
VIII = vertigo, unilateral sensorineural hearing loss, unilateral tinnitus
V = absent corneal reflex
VII = facial palsy
