MoA
Transom is an analgesic. it has multiple actions with the central nervous system, including opiate receptor stimulation and inhibition of the re-uptake of noradrenaline and serotonin.
Indications
Moderate to severe pain. Particularly if an opiate and/or ketamine is not going to be administered.
Contraindications
Known Severe allergy.
Aged less than 12 years
Cautions
- Taken within the last 4 hours.
- Abdominal pain. particularly if the patient is unwell and/or vomiting.
- aged >75 years particularly if there is a history of confusion or dementia.
- Confusion. Tramadols antichlolinergic activity may worsen confusion.
- pregnancy, should usually be withheld
Dosage
50mg PO
Onset and Duration
30-60 minute onset
4 - 8hr duration
Adverse affects
Nausea and/or vomiting
lightheadedness or feeling unusual
Sedation
dry mouth
Pharmacokinetics
metabolised in the liver and excreted by the kidneys
Common interactions
can cause serotonin syndrome when administered in combination with other serotonin increase recreational or prescribed drugs. e.g. tricyclics, ecstasy.
