What factors will reduce the number of clasps required for retaining the denture?

A

Reducing Clasping - i.e. need to increase retention elsewhere:
· Number of guide panes
· Soft tissue coverage
· Use of soft tissue undercuts i.e path of insertion
· Use of indirect retainers
· Effective clasps
· Correct occlusion

How can you change the appearance of the denture flange?

A

· Flange vs socketed
· Acrylic shade matching
· Stippling vs smooth to break up the ligh

What are the ideal conditions for taking a shade?

A

Always take the shade at the start of the appointment- to prevent incorrect shade recording due to dehydration of the teeth while mouth is open/rubber dam in place.
2. Teeth should be clean of debris and stain.
3. Room should be neutral colour and background should not be bright
4. Patient- no lipstick if bright and clothes draped with grey/neutral colour
5. Patient at eye level and directly in front of operator at arms length. Uses most cones in eye for best acuity.
6. Shade comparisons in different lighting conditions
7. Shade comparisons made quickly due to retinal fatigue – 5 seconds with tab under lip and adjacent to tooth being matched. After 5 seconds the eye accommodates and becomes biased.
8. Select value, hue and Chroma. (Although differing operators will use differing techniques).
9. View teeth wet and dry to avoid metameric effects
10. Observe contra-lateral side, adjacent and opposing teeth so it is integrated with the whole mouth.
11. Separate the teeth into 3 sections- enamel, dentine, pulp, gingiva as they all have differing colours
12. Magnification will improve accuracy theoretically!

A

What are the clinical and laboratory procedures involved with adding a tooth to a denture?

A

Take a cast impression before tooth extraction.

Cut off the tooth to be extracted from the cast.

Trim the ridge area to the recorded pocket depth to simulate post-extraction bone level.

Add the denture tooth in the correct shade and mould.

Can be added with a flange (more support) or socketed/flangeless (more aesthetic).

A

Chairside addition can also be carried out but takes much more clinical time
