Q: You are a medical student taking a history and examining a patient who has recently been diagnosed with Addison’s disease.

What symptoms and signs would you expect him to have presented with? (4)

A

A: -low bp (comes from insufficient aldosterone levels)

  • tanned (MSH increases)
  • tired (too little cortisol produced)
  • vitiligo (pale patches of skin= autoimmune)

What tests should be performed regarding her polyuria and polydipsia?

A

A: measure glucose levels

Over the last few weeks, she has had progressive weakness, affecting her thighs, with difficulty climbing stairs.

What is this condition called? How would you examine the patient for this?

A

A: proximal myopathy

Q: A 55-year old lady complains that she has been increasing in weight over the past two years. She also has a recent history of high blood pressure. Three months ago, she developed polyuria and polydipsia.

Over the last few weeks, she has had progressive weakness, affecting her thighs, with difficulty climbing stairs.

On direct questioning she notes that the shape of her face has changed and that she has gained a lot of weight ‘around her tummy’. She also mentions that she bruises easily, and that a wound on her left shin that she had six months ago has not healed.

What clinical signs would you expect to find on examination? (5) What do you think it is?

A

A: could have cushings- high cortisol, could act as aldosterone

  • HT
  • stretch marks are purple
  • round face
  • increased mid tummy size
  • pale skin

Q: Why is it important to make sure you take a thorough medication history in a patient with cushings?

A

A: lots of medication contains steroids which could cause/exacerbate symptoms

eg asthma inhalers
