Sally and Benjamin come into the pharmacy. Sally explains Benjamin has had an upset tummy since yesterday and is feeling ghastly. It is now 8:30 am.

B) I have had a few vomits overnight and my tummy feels a bit squeamish. I feel like I could vomit again though I haven’t since the early hours of this morning.
C) Started late yesterday afternoon, I wondered if it was the lunch we ate out at a cafe, though everyone else seems ok, it could be the salmon I had.
D) I drank some lemonade, I felt nauseous after drinking it
E) I take madopar (levodopa and benzerazide) for Parkinson disease and just some claratyne (loratadine) tablets if I ever need something for my nose.
F) Parkinsons disease and occasional hayfever
G) Just some plants, I get hayfever around springtime. Not serious,just a bit of runny nose.
Appropriate extended questions
- Are they are any other symptoms such as fever, diarrhea or abdominal pain?
- Have you had thise before
- How long have you been on the madopar
- Have you been traveling in the last few months, particularly overseas?
- Has anything changed recently?
- Couple of runny stools though nothing that i am too concerned and do not really want to take tablets to bind me up. Besides general nausea and discomfort when I actually vomit no other specific symptoms.
- I feel nauseous occasionally with my Parkinsons tablets, although this time the vomiting and nausea is different, I havent it had it for years.
- For about 2 years now, my dose was increased about 8 months ago.
- No only dunsbourough a couple of months ago
- The only thing is eating at this cafe yesterday we haven’t been to before. I think it will be our first and last meal there.
What is the diagnosis? Explain reasoning to the patient.
Based on the signs and symptoms you have told me, I believe you have gastroenteritis or more specifically acute vomiting. The runny stools along with the vomiting confirm my diagnosis. It is most likely triggered by the food you at the cafe which may have had a virus. It will go away soon as its the body’s response to get rid of the virus. I can give you medication to help with some of the symptoms.
What product would you recommend for this patient? Provide IDARL.
Gastrolyte
- Indication: Will replace electrolytes, salts, and sugars that you may have lost during vomiting and to prevent dehydration
- Dosage and directions: Dissolve sachet in 200mL of water. Sip in small amounts after vomitting. (use for 1-2 days). Discard after 24 hours once refrigerated.
- Adverse effects: generally well tolerated
- Duration of treatment, time to recovery, when to refer: Will take around 1-2 days to recover. I will refer you if there is blood your stools and if you cant keep fluid down. (also refer you if occurs longer than 3 days)
- Lifestyle/self care: don’t drink any more lemonade, doesn’t have the right amounts of sugar. Avoid solid foods and overeating until you stop vomiting. Slowly introduce dairy and meat into your diet.
